212. 1 Which component is physically damaged?
212. 2 Please describe the damage.
212.3 Where there contributing factors to the damage?
212.4 If receiver, ask the following:
          212.4.1 Is there any discoloration on the receiver screen?
212.5 If sensor/transmitter/wearable, ask the following:
          212.5.1 Has the transmitter/wearable housing been torn away from the adhesive patch?
          212.5.2 G6 / D1 only: Has one or both black contact points missing or become loose?
          212.5.3 G6 / D1 only: Has the latch that secures the transmitter onto the transmitter holder broken before or during deployment such that the transmitter cannot be properly installed?
          212.5.4 G6 / D1 only: Is a portion of the transmitter holder seated at approximately a 45-degree angle?
                       212.5.4.1 Can the patient collapse it down onto the transmitter holder?
                       212.5.4.2 G6 / D1 only: Is there a metal tube sticking out of it?
212.5.1 If 212.5.2/ 212.5.3 / 212.5.4 is “Yes”, can you provide a picture?
