233.4 Was there a labeling issue?
         If Yes:
         233.4.1 What was the labeling issue?
233.5 Was a component missing from the shipment?
         If Yes:
         233.5.1 Which component was missing?
233.6 Was the packaging damaged upon receipt.
         If Yes
         233.6.1 Describe the packaging damage.

1. If no, record the product expected and the product received and book the correct product with signature required and reference original SR. If missing return label, email one to patient. If yes, move to next question.
